Clinical : Utilizes nursing process in providing patient as follows :
4.1 Completes initial assessment of patient.
4.2 Prioritized and established wide range of specific problems which can be
alleviated by nursing intervention.
4.3 Revised patient care using initial and reassessment tool.
4.4 Implements patient care plans.
4.5 Evaluates patients response to treatment.
4.6 Correlates theory with practice in managing patients.
4.7 Participates in discharge planning of all identified patients for the Home Health
Care.
4.8 Facilitates the issuing of the required equipment and supplies for Home Health
Care patients.
4.9 Sets nursing objectives for all Home Care Patients.

4.10 Conducts audit activities in order to assess the quality of care offered.
4.11 Facilitates open channels of communication with all Home Health Care team
members.
4.12 Works towards quality improvement of the care provided.
4.13 Provides feedback to the team members and the Nursing head Nurse/Charge
Nurse, Nursing Supervisor, Director of Nursing, Asst. Director of Nursing, Deputy
director of Nursing.
4.14 Keeps records and updated documentation of patients on the Home Health Care.
4.15 Participates in Health Education Programme.
4.16 Participates in Nursing Education and staff Development.
4.17 Organizes patient visit schedule as per individual patient discharge plan and
monitor its implementation.
4.18 Participates in Home Health Care Team meetings and give feedback to the
Director of Nursing as required.
4.19 Carry out any additional duties as expected and within the scope of the Job
Description.
4.20 Updated BLS certification, Saudi Council License, Indication competency and
other staff development

1- Preferably Computer Literate.
2- Basic skills in specimen collection and other procedures related to area of
responsibility
