6.1 Actively participate in patient care rounds with other members of the healthcare team
6.2 Assist medical staff with the selection of drug therapy and patient monitoring.
6.3 Providing In-patient dispensing and counselling services.
6.4 Providing Out-patient dispensing and counselling when necessary.
6.5 Review and interpret medication orders for clarity, accuracy, rationality, compatibility, feasibility, and suitability for the patient's specific needs.
6.6 Assume responsibility for patient-specific pharmacotherapy plan to ensue e optimal Otltconies.
6.7 Act as drug information and education resource for patients, students, colleas tl es, medical and nursing start. and other healthcare professionals.
6.8 Conduct daily systematic medication reviews for patients in other floors other than the floor covered by the clinical pharmacist, whenever assigned.
6.9 Document thcrapeutic recorrlrnendations in the patient's medical record and pharmacy intervention forms.
6.10 Detect, analyse. and report adverse drug reactions.
6.11 Participate in the departmental performance improvement. educational. and other clinical activities.
6.12 Follow all Hospital related policies and procedures.
6.13 Participate in self and others' education, training and development, as applicable.
6.14 Perform other related duties as assigned.
6.15 Complies with drug laws and regulations, by monitoring nursing unit inspections; maintaining records for controlled substances: removing outdated and damaged drugs from the pharmacy inventory. supervising the work results of technicians and subpoint personnel; maintaining quitrent registration; studying existing and new legislation : anticipating legislation; advising management on needed actions.
6.16 Protects patients and staff by adhering to int“ection-control protocols. 6.17 Participate in antimicrobial stewardship, infection prevention control promoting the optimal use of antimicrobial agents reducing the transmission of infection & resistance
6.18 Maintains safe and clean working environment by complying with PSAFH procedures, rules. and regulations.
6.19 Maintains pharmacological knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
6.20 Contributes to team effort by accomplishing related results as needed.
6.21 Providing pharmacokinetic monitoring and dosing consultations to physician for drugs that have a narrow therapeutic index or high incidence of adverse effects.
6.22 Prompt conversion of IV medications to PO route whenever possible.
6.23 Providing patients with required nutritional support through TPN. when needed.
6.24 Permission for urgent mattes is requested from Pharmacy Director after filling up Time-In/Out form and specify how many hours you will consume outside the PSAFH.
6.25 To perform other appropriate tasks and duties that are assigned within their levels of skills, abilities, and knowledge
