المهام و الواجبات
1. Participates in the evelopment of departmental annual/strategic plans.
2. Facilitates the development and implementation of treatment guidelines, protocols and formulary changes as approved by appropriate committees.
3. Recommends alternative medications in response to medication shortages.
4. Participates in multidisciplinary clinical rounds/clinics.
5. Recommends evidence-based and cost-effective medication therapy regimens and monitoring plans
6. Participates in patient education and counseling sessions/programs.
7. Monitors medication therapy regimens for allergies, contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, and adverse drug reactions, appropriateness of drug and dose, and therapeutic duplications.
8. Assists in detecting, analyzing and reporting medication errors and adverse Drug Reactions (ARD's) in order to minimize/prevent them. (in coordination with Pharmacovigilance and Medication safety coordinator)
9. Provides accurate, adequate, and timely drug information to the professional staff (in coordination with drug information center supervisor/staff)
10. Prepares drug reviews, MUE criteria, antibiotic sensitivity reports and pharmacy interventions summary and present them to the P&T committee.
1 1 . Documents all clinical Pharmacy activities.
12. Develops and reviews policies and procedures for clinical Pharmacy activities with local and international standards.
13. Participates in the Quality improvement activities of the department.
14. Serves a preceptor for undergraduate and postgraduate pharmacy students.
1 5. Conducts and participates in continuing educational programs and training for pharmacy staff in coordination with the training and staff development coordinator.
16. Conducts in-service education to physicians, nurses and other health professionals of drug/disease related topics.
17. Represents the Pharmacy department in different multidisciplinary activities, scientific meetings and committees.
18. Assists in conducting and participating in clinical research studies and patient care audits.
19. Perform other applicable tasks and duties relevant to his/her knowledge, skills and abilities.

6. يشارك في تثقيف المريض وجلسات الإرشاد / البرامج.
7. يراقب نظم العلاج الدوائي للحساسية ، وموانع الاستعمال ، والتفاعلات الدوائية ، والتفاعلات الدوائية والغذائية ، وتفاعلات الاختبارات المعملية الدوائية ، والتفاعلات الدوائية الضائرة ، ومدى ملاءمة الدواء والجرعة ، والازدواجية العلاجية.
8. يساعد في اكتشاف وتحليل والإبلاغ عن أخطاء الدواء وردود الفعل السلبية (ARD) من أجل تقليل / منع حدوثها. (بالتنسيق مع منسق السلامة التيقظ الدوائي والأدوية)
9. تقديم معلومات دقيقة وكافية وفي الوقت المناسب عن الأدوية للموظفين المهنيين (بالتنسيق مع المشرف / الموظفين في مركز معلومات الأدوية)
10. يعد مراجعات الأدوية ومعايير MUE وتقارير حساسية المضادات الحيوية وملخص التدخلات الصيدلانية وتقديمها إلى لجنة P & T.
1 1. توثيق جميع أنشطة الصيدلة السريرية.
12. يطور ويراجع السياسات والإجراءات لأنشطة الصيدلة السريرية مع المعايير المحلية والدولية.
13-يشارك في أنشطة تحسين الجودة للإدارة.
14. يخدم كمدرس لطلاب الصيدلة الجامعيين وطلاب الدراسات العليا. 1
5. إجراء والمشاركة في البرامج التعليمية المستمرة والتدريب لموظفي الصيدلة بالتنسيق مع منسق التدريب وتطوير الموظفين.
16. تجري تثقيفًا أثناء الخدمة للأطباء والممرضات وغيرهم من المهنيين الصحيين حول الموضوعات المتعلقة بالمخدرات / الأمراض.
17. يمثل قسم الصيدلة في مختلف الأنشطة المتعددة التخصصات والاجتماعات واللجان العلمية.
18. يساعد في إجراء والمشاركة في دراسات البحوث السريرية ومراجعات رعاية المرضى.
19. أداء المهام والواجبات المطبقة الأخرى ذات الصلة بمعرفته ومهاراته وقدراته.

المهارات
1 Board Certification from the Board of Pharmacy specialties (BPS)
2 A minimum of 2 years of experience as clinical Pharmacist in 200+ beds tertiary/Specialized hospital (All experiences must be after the completion of the postgraduate program).
3 Ability to work independently and handle stress appropriately.
4. Ability to supervise and work cooperatively with pharmacy and other departments staff.
5. Ability to handle frequent interruptions and adapt to changes in workload and work schedule.

2 خبرة لا تقل عن سنتين بصفته صيدليًا إكلينيكيًا في أكثر من 200 سرير بمستشفى جامعي / متخصص (يجب أن تكون جميع الخبرات بعد الانتهاء من برنامج الدراسات العليا).
3 القدرة على العمل بشكل مستقل والتعامل مع الإجهاد بشكل مناسب.

المؤهلات العلمية
الماجستير في الصيدلة السريريى او شهادة الاختصاص الدقيق البورد في الصيدلية
