-Deliver excellent nursing care, monitors, maintain standards of care within the F&CM
policy and procedures.
-Follow the nursing standards, and apply evidence-based approach for the assessment,
planning, implementation and evaluation of patient care.
-Identify and act on changes in patient condition, making safe, timely decisions
appropriate to the situation.
-Be responsible for the appropriate assessment, planning, implementation of safe,
effective nursing interventions.
-Document and evaluate nursing interventions.
-Assist case managers in liaising between healthcare professionals, doctors and allied
health professionals to ensure appropriate medical care provided to the patient.
-Ensure appropriate consent obtained and maintained for patient undergoing relevantmedical treatment.
-Ensure the safe administration and management of medication.
-Advocate on behalf of their patients to ensure quality health care services provided.
-Be responsible for family / caregiver training about patient care provision.
-Maintain and protect the safety, confidentiality and dignity of patients and families.
-Manage a clean, safe and tidy workplace, vehicles, and team bags for F&CM patient
home visits.
-Maintain privacy and confidentiality of all patient related information in relation to
service provision and health care treatments and services.
2. Leadership and Management:
-Work with Colleagues to provide support to all staff of the F&CM team.
-Contribute to the professional development of peers, colleagues and others.
-Recognizing and valuing contributions from individuals with in the team.
-Provide Clinical Support.
-Working with team members to define clear individual roles and responsibilities.
-Ensure all written or verbal information is clear, accurate and free from jargon.
-Demonstrate effective communication and promote good interpersonal relationships in
work the place.
-Promote a culturally and religiously congruent environment conducive to successful
learning outcome
-Handle conflicts and misunderstanding in a positive manner and a satisfactory
resolution achieved.
-Deal with complaints promptly and sensitively with appropriate documentation and
follow up reports with the charge nurse.
-Assume accountability for quality care delivered to the patients.
-Take charge in the absence of the person responsible by demonstrating leadership and
management skills as assigned.

-Implement the vision and mission statement and standards of home health care.
-Monitor the use of stock items and order as necessary.
3. Professional Development:
-Participate in teaching programs and Continual Professional development (CPD)
educational training within Home Health Care, FCM
-Take responsibility for own professional development.
-Act as preceptor for new nurses and students.
-Act as a professional role model and mentor for junior team members.
-Provide health education to the patients, relatives and caregivers.
-Maintain generic and unit specific competencies up-to-date.
-Demonstrate safe and correct use of equipment.
4. Quality Improvement:
-Participate in clinical audits and quality improvement Initiatives.
-Lead in quality improvement and facilitation of change when necessary.
-Participate in the implementation and evaluation in action plan for performance
improvement.
-Comply with departmental and hospital policies and procedures and participate to
develop policies, procedures and required protocols in collaboration with relevant
nursing leaders and managers.
-Perform other applicable tasks and duties assigned within the realm of the employee's
knowledge, skills and abilities.

- Work with colleagues to provide support to all ward staff .

- Contribute to raising the profile and development of the nursing team by providing clinical support .
