Duties and Responsibilities
Visits the wards regularly and establishes relationships with the medical and nursing staff.
Interviews patients and their family members and friends to develop background knowledge about the patient’s social life.
Carries out home visits to follow-up the progress of cases.
Registers cases in the department for reference and future review.
Holds staff meetings to discuss cases interviewed, and plans that may be followed to treat them.
Contacts community social services with formal letters to enlist their help and cooperation in the treatment of some of the cases.
Participates in CQIPS System Improvement Activities, System Control Activities, Staff Development Activities.
Performs other applicable tasks and duties assigned within the realm of his/her knowledge, skills, and abilities as required.
