.2.1. Support the Mission, Vision and Values of the Department of Pharmacy.

2.2. Participate in interdisciplinary patient care rounds and communicate follow up with clinical team.

2.3. Monitor Drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication and drug interactions.

2.4. Prevent, detect and resolve medication related problems and to make appropriate evidence based, patient centered medication therapy recommendations as part of the interdisciplinary team and document therapeutic recommendations in the multi disciplinary plan of care sheets and clinical Pharmacist intervention form.

2.5. Provides drug and clinical information to other health care professionals in response to specific questions.

2.6. Provides and document discharge medications education.

2.7. Actively participate and the development and implementation of performance/quality improvement and patient safety activities for clinical service area and pharmacy.

2.8. Participates in clinical guidelines development disease management and drug use programs.

2.9. Prevent, detect, and Monitor, Document and report adverse drug reactions and medications errors.

2.10. Provides managerial responsibilities for the operations of the pharmacy including process improvements, competing performance reviews for pharmacy staff.

2.11. Supports the education and professional development of the pharmacy department.

2.12. Designs, supports and implements policies to provide effective and efficient use of resources and for ensuring that the budget, quality and value the targets are met in collaboration with the Director of Pharmacy.

2.13. Participates in committees including but not limited to Pharmacy and Therapeutics Committee, CQI and PS , Infection Control etc.

2.14. Performs other related duties as assigned by the Director of Pharmacy.

2.5. يقدم معلومات عن الأدوية والسريرية لأخصائيي الرعاية الصحية الآخرين ردًا على أسئلة محددة.

2.6. يوفر وثيقة التعليم الأدوية التفريغ.

2.7. المشاركة بنشاط وتطوير وتنفيذ أنشطة تحسين الأداء / جودة وسلامة المرضى لمنطقة الخدمة السريرية والصيدلية.

2.8. يشارك في المبادئ التوجيهية السريرية لإدارة الأمراض وتطوير برامج تعاطي المخدرات.

2.9. منع واكتشاف ومراقبة وتوثيق والإبلاغ عن ردود الفعل السلبية للأدوية والأخطاء الدوائية.

2.10. يوفر المسؤوليات الإدارية لعمليات الصيدلية بما في ذلك تحسينات العملية ، ومراجعات الأداء المتنافسة لموظفي الصيدلة.

2.11. يدعم التعليم والتطوير المهني لقسم الصيدلة.

2.12. تصمم وتدعم وتنفذ سياسات لتوفير استخدام فعال وكفء للموارد ولضمان الوفاء بالميزانية والجودة والقيمة المستهدفة بالتعاون مع مدير الصيدلة.

2.13. يشارك في اللجان بما في ذلك على سبيل المثال لا الحصر لجنة الصيدلة والعلاج ، CQI و PS ، مكافحة العدوى ، إلخ.

2.14. يؤدي واجبات أخرى ذات صلة كما تم تعيينها من قبل مدير الصيدلة.

.3.1. A degree in Pharmacy, B.Sc, B. Pharm and Masters in clinical Pharmacy.
3.2. Two year experience in Hospital IP Pharmacy as Clinical Pharmacist in minimum 200 bed Hospital.
