صيدلية سريرية (Clinical Pharmacist)

1- Participates in multidisciplinary clinical rounds/clinics.
2- Recommends evidence-based and cost-effective medication therapy regimens and monitoring plans.
3- Facilitates the development and implementation of treatment guidelines, protocols and formulary changes as approved by appropriate committees.
4- Recommends alternative medications in response to medication shortages.
5- Participates in patient education and counseling sessions/programs.
6- Assists in detecting, analyzing and reporting medication errors and adverse Drug Reactions (ARD’s) in order to minimize/prevent them. (incoordination with Pharmacovigilance and Medication safety coordinator)
7- Provides accurate, adequate, and timely drug information to the healthcare providers.
8 -Perform Therapeutic Drug Monitoring (TDM) for all needed medication.
9 -Participating in reconciliation process at admission, transfer of care &discharge.
10- Prepares drug reviews, MUE criteria, antibiotic sensitivity reports and pharmacy interventions summary and present them to the P&T committee.
11- Documents all clinical Pharmacy activities.
12- Participates in the quality improvement activities of the department.
13 - Serves as a preceptor for undergraduate and postgraduate pharmacy students.
14- Conducts and participates in continuing educational programs and training for pharmacy staff in coordination with the training and staff development coordinator.
15- Conducts in-service education to physicians, nurses and other health professionals of drug/disease related topics.
16- Represents the Pharmaceutical Care Services in different multidisciplinary activities, scientific meetings and committees.
17- Assists in conducting and participating in clinical research studies and patient care audits
18- Perform other applicable tasks and duties relevant to his/her knowledge, skills and abilities.

٢ يقدم التوصيات العلاجية المبنية على الدليل ويقوم بمتابعة تنفيذها ونتائجها
٣ يساعد في تطوير وتطبيق التوصيات والبروتكولات العلاجية وتعديلات قائمة أدوية المستشفى طبقا لقرارات اللجان ذات الصلة.
٤ يوصي بأفضل البدائل المتاحة في حالة نقص الأدوية.
٥ يقوم بتعليم المرضى وإرشادهم فيما يتعلق بالاستخدام الأمثل للأدوية.
٦ يساعد في اكتشاف، تحليل، والإبلاغ عن الأخطاء الدوائية والأعراض الجانبية للأدوية (بالتنسيق مع منسق السلامة الدوائية).
٧ يقدم المعلومات الدوائية الدقيقة والمحدثة للممارسين الصحيين وفقا لاحتياجات المرضى.
٨ يتابع مستوى الأدوية ذات المجال العلاجي الضيق في الدم ويقدم توصيات تعديل الجرعات بناء على ذلك.
٩ يشارك في عمليات الملائمة الدوائية عند بداية التنويم، نقل المرضى من قسم لآخر، وعند الخروج من المستشفى.
١٠ يقوم بعمل مراجعات علمية لفعالية وأمان الأدوية ومدى الحاجة لها، وكذلك مراجعة لاستخدامها الفعلي داخل المستشفى، كما يشارك في إعداد تقارير فعالية المضادات الميكروبية والتوصيات الصيدلانية وتقديمها للجنة الصيدلة والعلاجيات.
١١ يسجل كافة أنشطة الصيدلة الإكلينيكية من خلال الأنظمة المعدة لذلك.
١٢ يقوم بتدريب طلاب كليات الصيدلة وبرامج الدراسات العليا.
١٣ يشارك في تقديم محاضرات علمية فيما يتعلق بالممارسة الصيدلانية والمستجدات الدوائية لمنسوبي الصيدلية والممارسين الصحيين بشكل عام (بالتنسيق مع منسق التدريب والتطوير).
١٤ يمثل إدارة الرعاية الصيدلانية في اللجان والاجتماعات العلمية والمهنية الملائمة لتخصصه والوصف الوظيفي
١٥ يشارك في إجراء الأبحاث الإكلينيكية والممارسة الصيدلانية كلما أمكن ذلك.
١٦ يقوم بآداء المهام والواجبات الأخرى المكلف بها بما يتناسب مع معارفه، خبراته، وقدراته

1 Ability to work independently and handle stress appropriately, demonstrates leadership In the practice of pharmacy, ability to foster a cooperative work environment.
2 Strong interpersonal and communication skills and the ability to work effectively With branch departments ability to set priorities. makes critical decisions, and responds quickly to emergency requests
3 Board Certification from the board Pharmcy specialties/experience in paediatric pharmacotherapy or Infectious diseases is preferred or  critical care.

1 القدرة على العمل بشكل مستقل والتعامل مع التوتر بشكل مناسب، وإظهار القيادة في ممارسة الصيدلة، والقدرة على تعزيز بيئة عمل تعاونية.2 مهارات قوية في التعامل مع الآخرين ومهارات الاتصال والقدرة على العمل بفعالية مع قدرة الأقسام الفرعية على تحديد الأولويات.يتخذ قرارات حاسمة، ويستجيب بسرعة لطلبات الطوارئ3 - يفضل شهادة البورد من تخصصات  او  الصيدلة / الخبرة في العلاج الدوائي للأطفال أو الأمراض المعدية او الرعاية الحرجة.
