Duties and Responsibilities
1 Advise medical staff on the prescribing of therapeutic diets and nursing staff on the nutritional management and administration of dietary requirements.
2 Visits patients at home and develops a Plan of Care.
3 Perform an evaluation / assessment of each patient's unique dietary needs during the first visit.
4 Following referral from a medical practitioner, to interview and instruct patients and their relatives/caregivers on the management of dietary treatment at home.
5 Interview patients or caregivers as required to assess their nutritional intake/status and advise on specific dietary requirements.
6 Assist in research projects and relevant education of medical, nursing, and catering staff.
7 Assist with the production of teaching aids and diet sheets required by the Department.
8 Liase with Catering Department.
9 Participate in Environmental Safety Rounds.
10 Responsible for keeping updated in Infection Control, Fire and Safety and Disaster Training on a yearly basis.
11 Participate in the Home Health Care quality improvement activities.
12 Promote health education to patient and their family.
13 Participate in CQI & PS Activities.
