Duties and Responsibilities
5.1 Adhere to all policies and procedures implemented by the pharmacy department.
5.2 Assume responsibility of area supervisor when required
5.3 Efficiently and accurately verify physicians order, resolve drug interactions, and verify appropriateness of drug, dose, and rout of administration
5.4 Recommend suitable medications to physician and nursing staff, tailored to individual patient needs, safety, formulary restrictions, and ease of administration
5.5 Conduct drug information and literature searches for staff physicians
5.6 Communicate with patient, physicians and other healthcare professionals regarding drug-therapeutic duplications.
5.7 Perform audits of ward medication rooms and crash-carts to ensure properness of store medication.
5.8 Consult patients regarding their prescription
5.9 Check of new prepared, refilled prescriptions and pharmacy related products (extemporaneous preparations) before dispense it to patients
5.10 Train, develop and supervise pharmacy technicians and pharmacy student staff on operations of pharmacy
5.11 Apply evidence-based medicine within pharmacy operations.
5.12 Make clinical interventions to physicians regarding appropriate drug therapy
5.13 Document and perform adverse drug reactions and medication errors pertaining to medication management and safety
5.14 Resolve problems within their scope of practice
5.15 Responsible for compounding, packaging, and labeling of IV medication
5.16 Complete any calculation of product preparation
5.17 Compound sterile admixture and no-admixture induces TPN, antibiotics, chemotherapy using aseptic technique and USP 797
5.18 Ensure clean room and related equipment is clean sanitized and maintained to compliance with hospital policy and procedure
Skills
7.0 PROFESSIONAL EXPERIENCE:
7.1 Essential: 2 years of continuous working experience in a hospital
7.2 Desirable: Pharm D certification
